Care Coordinators work closely with primary care providers, Early Intervention staff, Early Childhood Special Education staff, Maternal Child Health, WIC, Office of Childcare, DWS, The Children’s Center, and other community agencies or community providers to ensure a smooth referral process for families.

Rent to Own Program: CROWN Program
Allows people to pay rent towards a home for 15 years while earning credit to purchase the home in the future.
The payments toward the home are cheaper in the long-run than paying for a home.

Must make under 200% of the federal poverty level.
Must have at least one person in the home who is a US citizen.
Must apply for HEAT before applying for the weatherization program.
