Notice of Privacy Practice (HIPPA)

Your Privacy Is Important to Us
At Jasper Counseling and Wellness, LLC, we understand that your mental health information is deeply personal. We are committed to protecting the privacy and confidentiality of your records. This Notice of Privacy Practices describes how your protected health information (PHI) may be used and disclosed, and how you can access this information. This notice is provided in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and the laws of the State of Utah.
How We Use and Share Your Information
We may use your health information in the course of providing you with therapeutic services. For example, your information may be shared among your treatment providers to ensure that you are receiving coordinated and effective care. If we need to consult with another provider, such as a psychiatrist, or refer you for additional services, we may share relevant information to support your treatment.
We may also use your information to bill for services. This can involve communicating with your insurance company, health plan, or other payer in order to receive payment for your sessions. In doing so, only the minimum necessary information will be shared.
In addition to treatment and payment, we may use your information for internal operations such as quality assessment, supervision, case consultation, and administrative tasks. These activities are designed to help us improve our services and ensure that you receive the best possible care.
There are times when we may be required by law to share your information without your consent. This may include situations where there is suspected abuse or neglect, threats of harm to yourself or others, court orders, or when required for certain public health activities. We follow all state and federal guidelines in these situations and only share what is legally necessary.
Your Rights as a Client
As our client, you have several important rights when it comes to your health information. You have the right to request a copy of your treatment records, either electronically or in paper form. You may also ask us to correct information in your record if you believe something is inaccurate or incomplete.
If you prefer to be contacted in a specific way - for example, only by phone or only at certain times - you can request that we communicate with you confidentially. You may also ask us not to use or share certain parts of your health information for treatment, payment, or operations, although we are not always required to agree to such requests.
You are entitled to a list of disclosures that we’ve made of your information for purposes other than treatment, payment, or healthcare operations. You also have the right to receive a paper copy of this notice at any time, even if you originally received it electronically.
Our Legal Responsibilities
We are required by law to maintain the privacy and security of your protected health information. We must provide you with this notice and comply with its terms. If there is ever a breach of your health information, we will notify you in a timely manner, as required by HIPAA.
Occasionally, we may update this notice to reflect changes in our privacy practices. Any changes will apply to all the information we maintain and we will make the revised notice available to you upon request.
